This disclaimer is to be carefully read and signed by all clients and/or seminar/workshop participants.

It is understood that all participation is strictly voluntary.

All participants/clients are in full charge of their own healing, and therefore, release, any and all, responsibility, and /or liability, toward the Wellness Center, and/or, Angela Ann, Wellness Counselor. It is understood that this agreement is binding and all encompassing.

Compliance with this agreement is in effect as of date of first session and/or participation, and will be considered binding throughout, and for the duration of participation with the Wellness Center, and/or, Angela Ann, Wellness Counselor.

All seminar fees and initial consultation fees are to be paid in full before session begins. Payment for individual consultations are to be submitted immediately following the session.

If signed, the participant/client agrees to the above in full knowledge and of free will.
